We have 35 Community Aged Care Packages (CACPS).
Community Aged Care Packages are designed for older adults who wish to remain living at home but would require several services to help them do so.
We can help you access and coordinate services to meet your individual needs. Individual care plans are developed with the person in consultation with the family, and can be adjusted as needs change.
The types of services provided may include:
- Case management
- Meals delivery
- Assistance with personal hygiene
- Prompting medications
- Housework
- Transport to appointments
- Shopping
