Our aged and disability services is a Designated Home and Community Care Assessment Agency, responsible for implementing an Active Service Model.
The Assessment and Care Planning Officer is responsible for conducting a living at home assessment aimed at:
- Understanding clients and carers needs, broadly defined opposed to interpreting needs only in relation to the services that can be provided by the council,
- Identifying opportunities for maximising functional capacity and early intervention, and
- Reducing occupational health and safety risks for clients, primary carers and staff.
