Aged Care Navigation Support
Many older people may be eligible for services such as personal care, domestic assistance and help with shopping. However, accessing aged care services can feel confusing.
We're here to make it easier for you and your family.
Council’s Aged Care Navigation Officer offers free and impartial navigation support over the phone, online or through a face-to-face visit.
Our officer can assist you with:
- explaining aged care services and eligibility
- registering with My Aged Care
- finding aged care providers and understanding referral codes
- and providing information about other related services, such as the half price taxi card.
Our services are free, impartial, and tailored to your individual needs.

Positive Ageing Care Finder
Positive Ageing Care Finders support vulnerable older people who would not be able to arrange aged care services without intensive support and do not have a family member or friends who can help. Positive Ageing Care Finder is delivered in partnership with City of Darebin and Merri-bek City Council.
We can support you to connect with My Aged Care so your needs can be assessed to determine the services you may be able to access. Positive Ageing Care Finders can also support you to connect with service providers of the Commonwealth Home Support Program (CHSP), Support at Home (SAH) and residential care. Some examples of aged care services include domestic assistance, support with showering and dressing, assistance with shopping, meals delivered to your home, social and exercise programs, and community transport.
We can also help you access other community supports such as health services, mental health services, drug and alcohol supports, housing and homelessness services and community groups.
Positive Ageing Care Finders are not for everyone. They specifically help vulnerable older Australians who need intensive assistance to access aged care and other supports.
To receive support, a person must:
- have no carer or support person who can help them, or
- not have a carer or support person they feel comfortable or trust to support them, and
- be eligible for government-funded aged care.
In addition, they should have one or more of these reasons for needing intensive support:
- have difficulty communicating because of language or literacy
- find it difficult to understand information and make decisions
- be reluctant to engage with aged care or government
- be in an unsafe situation if they do not receive services.
Positive Ageing Care Finders help you understand the Aged Care system and link with services that may be available to you.
We can talk about your needs and situation to find services that help you stay healthy, safe and connected to your community.
Positive Ageing Care Finder is free and confidential.
A Positive Ageing Care Finder will meet with you in your home or another place you choose. You can refer yourself to a Positive Ageing Care Finder or refer someone else as long as you have their consent.

Changes to in-home Aged Care
Since 1 November 2025, the Support at Home program has replaced the existing Home Care Package. The Commonwealth Home Support Program (CHSP) will transition to the Support at Home program no earlier than 1 July 2027.
