If you are not currently a client of Moonee Valley City Council, please contact My Aged Care or call on 1800 200 422 to register your details and schedule an assessment.

To access services provided on this page, you have to be assessed by My Aged Care  for the CHSP or SAH program.

· Commonwealth Home Support Program (CHSP) : for older adults who receive entry-level support

· Support at Home (SAH) : for older adults who need more help to live safely and independently at home

Anyone who needs care and is aged 65 or older, or aged 50 or older if they are an Aboriginal or Torres Strait Islander person, or if they are homeless or at risk of homelessness, can have an aged care needs assessment by an approved assessor.

If you’re under 65 and you have additional support needs (Aboriginal and Torres Strait Islander who are under 50), you could be eligible for the Home and Community Care Program for Younger People (HACCPYP). Find out more on the Accessible Moonee Valley page.

Seniors services navigation support

We're here to make it easier for you and your family.

Council’s Aged Care Navigation Officer offers free and impartial navigation support over the phone, online or through a face-to-face visit.

Our officer can assist you with:

  • explaining aged care services and eligibility
  • registering with My Aged Care
  • finding service providers and understanding referral codes
  • and providing information about other related services, such as the half price taxi card.
