Apply for an assessment with My Aged Care
If you’ve realised that you need some help and want to access aged care services, the first step is to get assessed.

Accessing services
To access services provided on this page, you have to be assessed by My Aged Care for the CHSP or SAH program.
· Commonwealth Home Support Program (CHSP) : for older adults who receive entry-level support
· Support at Home (SAH) : for older adults who need more help to live safely and independently at home
Anyone who needs care and is aged 65 or older, or aged 50 or older if they are an Aboriginal or Torres Strait Islander person, or if they are homeless or at risk of homelessness, can have an aged care needs assessment by an approved assessor.
Fees
You may be asked to pay part of the cost of your care. This is called a participant contribution, sometimes referred to as client contribution. The amount depends on your personal situation, and you will be told about any costs before your care begins. Financial hardship assistance may be available if you are experiencing difficulty with payment.
Prices for services include the entire cost of delivering a service (e.g. administrative costs, staff travel and third-party charges).

Domestic assistance
We help you keep your home clean, safe and comfortable.
- Vacuum and mop
- Clean bathrooms and toilets
- Wash dishes and clothes
- Light ironing and meal preparation
- Shop for you using your list
- Pay bills without going with you
- Change bed linen.
We only clean areas you use
You must be home for shopping deliveries
Give cash only — never your bank card
You will get a receipt and your change
Clean windows or dust (unless for health reasons)
Sweep outside or use ladders
Move heavy furniture
A mop, broom and vacuum
Safe cleaning products from our approved list
Assistance with self-care and daily living
We help with daily tasks so you feel clean, safe and comfortable.
Help with bathing or showering
Help with dressing and grooming
Support with toileting
Help getting in and out of bed
Support with eating meals
Remind you to take medicine
Help you move around your home
Put on support stockings
We do not do medical tasks. A nurse is needed for that.
Social and Community Engagement
We help you stay active and connected in your community. It helps you stay social, healthy and involved.
What we can do
- Go shopping with you
- Assist with paying bills
- Attend appointments with you
- Visit the library or community groups
Respite
We give your carer a break and help you enjoy time on your own.
Support you at home
Take you into the community
Provide care regularly or sometimes
It helps you and your carer stay well and supported.
Community meals and delivered meals
We deliver fresh, chilled meals to your home.
Cost: From $6.90 per meal.
- Tuesdays, Wednesdays and Fridays
- Between 9:30am and 1.30pm
- Nutritional main meal
- Delivered by Council staff
- Regular wellbeing check ins during our visits
- Support with storing your meals upon delivery and making sure you have enough food on hand.
- You must be home on your delivery day
- Meals are fresh and cannot be left unattended
- Meals can be picked up at the Ascot Vale Neighbourhood Centre
Home adjustments and maintenance
Our Home adjustments service can assist you to remain living independently and safely in your home by providing adjustments to your home that have been prescribed by an occupational therapist.
Our team of qualified and friendly staff will help work out your needs, making decisions together.
We can visit you and provide advice and a free quote.
Home adjustments include structural change to your home that increase or maintain your independence.
Our qualified team can install:
- handheld shower hoses
- shower stools
- grab/handrails (not modular)
- banister rails
- ramps
- platforms
- step extensions
- chair raisers
- safety hinges (not Ross)
- non-slip tape/strips
- remove shower screen/door and replace with a shower curtain
Home maintenance focuses on repairs or maintenance to support your wellness and mobility.
If you need repairs we can:
- install smoke alarms and/or replace batteries
- replace light globes
- replace tap washers (not flick mixers)
- replace fly screen on windows and doors
- repair doors
- repair/replace locks
- flip bed mattresses
- replace garden taps
- replace tap handles (not lever taps)

Additional services for SAH participants
In addition to the services listed above, Support at Home program participants may also be assessed as eligible to receive other services including:
- nursing care
- allied health and other therapeutic services, such as physiotherapy and podiatry
- home maintenance and repairs, including gardening
- assistive technology, such as personal alarms
- indirect transport, such as taxi vouchers.
The Home Care Packages (HCP) program transitioned to the Support at Home program (SAH) on 1 November 2025. HCP care recipients transitioned to Support at Home at their current funding levels.
People on the National Priority System awaiting allocation of a HCP will transition to the Support at Home Priority System and have a budget allocated once a place is available. They will retain an equivalent budget to their assessed HCP budget. They won’t need a new assessment unless their needs change.
It is critical that older Aboriginal and Torres Strait Islander people can access culturally safe, trauma-aware and healing-informed aged care in or close to their community. Support at Home is responsive to the diverse and changing needs of older Aboriginal and Torres Strait Islander people through:
- including culturally-safe care services on the service list – such as assistance to participate in cultural activities and access to Aboriginal and Torres Strait Islander health practitioners and health workers
- providing a supplement for extra hours of care management to providers that support older Aboriginal and Torres Strait Islander participants.
Seniors services navigation support
Council’s Aged Care Navigation Officer offers free and impartial navigation support over the phone, online or through a face-to-face visit.
