Understanding and accessing aged care services in Moonee Valley

 

Aged Care Navigation Support

Many older people may be eligible for services such as personal care, domestic assistance and help with shopping. However, accessing aged care services can feel confusing.

We're here to make it easier for you and your family.

 

Council’s Aged Care Navigation Officer offers free and impartial navigation support over the phone, online or through a face-to-face visit.

Our officer can assist you with:

  • explaining aged care services and eligibility
  • registering with My Aged Care
  • finding service providers and understanding referral codes
  • and providing information about other related services, such as the half price taxi card.

 

Our services are free, impartial, and tailored to your individual needs.

Positive Ageing Care Finder

Positive Ageing Care Finders support vulnerable older people who would not be able to arrange aged care services without intensive support and do not have a family member or friends who can help. Positive Ageing Care Finder is delivered in partnership with City of Darebin and Merri-bek City Council.

Changes to in-home Aged Care

The Department of Health and Aged Care announced that the new Support at Home program has replaced the existing Home Care Package (HCP) since 1 November 2025 and the Commonwealth Home Support Program (CHSP) will transition no earlier than 1 July 2027.
