1. I am free of COVID-19 symptoms including: Loss or change in sense of smell or taste, fever, chills or sweats, cough, sore throat, shortness of breath, runny nose.
2. I have not been in contact with a confirmed case of COVID-19.
3. I have not been directed to isolate in the last 14 day.
4. I will notify program staff of any changes and ensure I follow COVID-19 regulations and guidelines given by the Victorian Government
5. I will follow any program COVID-19 safety measures put in place to protect me in safely participating in the program.

Moonee Valley City Council (Council) is collecting your personal information in order to register you these programs.

Council may use the information collected for the secondary purpose of notifying you of updates to our programs, events and Portal and to inform our reporting obligations to Federal and State Government agencies. Your personal information will be stored in Councils database/s. Council will not disclose your personal information without your consent, except where required or authorised to do so by law.

You also agree to indemnify and keep indemnified and to hold harmless the Moonee Valley City Council, its servants, agents and contractors from and against all actions, costs, claims, charges, expenses and damages whatsoever, which may be brought or made against them or any of them arising out of or in connection with the Moonee Valley City Council Program except to the extent that Moonee Valley City Council, its servants, agents and contractors are liable at law.

When attending programs/ events, you agree to work within your own physical limitations at all times and inform of any change in your health, circumstances or medication that may affect your participation.

Your image/ name as part of the Program may be used by Council in promotional material to promote the program in print, broadcast or online media. Moonee Valley City Council.
